What is Collected

Body Rehab collects your personal information when you use the website and communicate by email or phone. This information can include your name, contact information, phone numbers and payment information.

We may also collect non-personal information such as usage as your IP address, browser version, operating system which may be released in anonymous form such as a report publishing our website usage trends.

Any credit card details are processed securely in a PCI compliant payment gateway using a secure HTTPS internet connection.

Like most websites, Body Rehab uses cookies to store a user's website preferences. If you do not wish to have cookies placed on your computer, you should set your browser to refuse cookies before using our website, with the drawback that certain features will not function properly without the aid of cookies.

How it is used

The personal information that is collected to provide is for billing, identification, authentication, service improvement, research and website development.

Body Rehab will not share your personal information to anyone except to provide the requested services, develop our products, protect our rights, assist with a lawful investigation, comply with the law, or to contact you.

Some third party service providers such as website hosts, payment service providers and backup service providers may have access to personal information held by us that a) need to know that information in order to process it on behalf of Body Rehab or to provide services available at the Body Rehab website and b) have agreed not to disclose it to others.

Aggregated non-personal data is collected by Body Rehab may be shared with third parties in order to improve the Body Rehab website.

Privacy and Consent

Body Rehab needs to collect information about you for the primary purpose of providing a quality service to you. In order to thoroughly assess, diagnose and provide therapy, we need to collect some personal information from you. If you do not provide this information; we may be unable to treat you.  This information will also be used for:

  1. The administrative purpose of running the practice;
  2. Billing either directly or through an insurer or compensation agency;
  3. Use within the practice if discussing or passing your case to another practitioner within the practice for your ongoing management;
  4. Disclosure of information to your doctors or other health professionals to facilitate communication and best possible care for you; and
  5. In the case of insurance or compensation claim it may be necessary to disclose and/or collect information that concerns your return to work to an insurer or your employer.

We do not disclose your personal information to overseas recipients.

To ensure the process of quality treatment provision, information about your assessment results and progress may be given to relevant other service providers, who are involved in your management. These may include your doctor, specialists, insurers, solicitors or employers. 

I have read the above information and understand the reasons for the collection of my personal information and the ways in which the information may be used and disclosed and I agree to that use and disclosure.

I understand that it is my choice as to what information I provide and that withholding or falsifying information might act against the best interests of my assessment and therapy progress.

I am aware that I can access my personal and treatment information on request and if necessary, correct information that I believe to be inaccurate.

I understand that if, in exceptional circumstances, access is denied for legitimate purposes, that the reasons for this and possible remedies will be made available to me.

Social Media

Body Rehab may tag you in a photo on Social Media or include your Facebook profile photo if you leave a testimonial. You can request for photo to not be displayed or shared.

Business Transfers

If Body Rehab was acquired, or in the unlikely event that it goes out of business or enters bankruptcy, user information would be one of the assets that is transferred or acquired by a third party. You acknowledge that such transfers may occur, and that any acquirer of Body Rehab may continue to use your personal information as set forth in this policy.

Although most changes are likely to be minor, Body Rehab may change its Privacy Policy from time to time, and at Body Rehab sole discretion.
